Ostoluxe provides specialized, advanced-practice ostomy care for patients with ileostomies and colostomies throughout Maryland and Delaware.
From preoperative preparation and stoma-site marking to rapid post-discharge follow-up, complex ostomy management, and lifelong specialty care, Ostoluxe is designed to help bridge the gaps that can occur between hospital, specialty, and outpatient care.
Our goal is simple:
Provide patients with timely access to specialized ostomy care while supporting—not replacing—the clinicians and healthcare teams who already care for them.
Why Refer to Ostoluxe?
The transition from hospital to home can be a vulnerable period for patients with a new ostomy.
Ostoluxe provides an additional specialty resource for these patients outside of the hospital setting.
Advanced-Practice Provider Care
Patients are evaluated by a doctorate-prepared nurse practitioner with specialized ostomy training and prescriptive authority, allowing assessment, treatment planning, medication management, diagnostic evaluation when appropriate, and ostomy supply prescribing to be integrated into the same plan of care.
Readmission-Risk Reduction
Particular attention is given to factors associated with preventable acute-care utilization, including high-output ileostomy, dehydration, medication-related concerns, nutritional challenges, and difficulty managing the ostomy at home.
Continuity Beyond the Initial Recovery
Ostoluxe remains available as an ongoing specialty resource as the patient's stoma, health, pouching requirements, and clinical needs evolve.
Who Should Be Referred?
Ostoluxe welcomes patients who may benefit from specialty ostomy evaluation or management, including those who are:
- Preparing for ileostomy or colostomy surgery
- In need of preoperative ostomy education or stoma-site marking
- Newly discharged following ostomy creation
- At increased risk for dehydration or readmission
- Experiencing high or difficult-to-manage ileostomy output
- Having recurrent emergency visits related to dehydration or ostomy concerns
- Struggling with pouch leakage, poor wear time, or appliance fit
- Experiencing peristomal skin complications
- Developing changes in stoma appearance, function, or anatomy
- Requiring assessment of a flush, retracted, prolapsed, or otherwise challenging stoma
- Experiencing nutritional, hydration, or intestinal adaptation concerns
- Requiring medication management related to ostomy function or output
- In need of pouching-system or accessory optimization
- Having difficulty obtaining appropriate ostomy supplies
- In need of new or updated ostomy supply prescriptions or orders
- Living with a longstanding ostomy and experiencing a new or recurring concern
- In need of ongoing specialty ostomy follow-up

A Focus on the Post-Discharge Gap
The days after discharge matter.
For selected patients, early specialty intervention may provide an opportunity to identify and address problems before they progress to emergency-level care.
Ostoluxe places particular emphasis on:
High-output ileostomy
Early recognition and individualized management of excessive output and associated fluid losses.
Dehydration risk
Assessment of hydration status, oral intake, medications, output, and other clinical factors that may contribute to dehydration or kidney injury.
Medication optimization
Evaluation and management of medications affecting ostomy output, intestinal transit, hydration, and absorption when appropriate.
Nutrition & intestinal adaptation
Individualized support for altered gastrointestinal anatomy and changing nutritional and hydration requirements.
Pouching failure & skin complications
Early intervention when leakage, poor appliance fit, or skin breakdown threatens successful ostomy self-management.
Access to supplies
Assistance with identifying appropriate products and obtaining the necessary prescriptions and orders.
The goal is not simply to troubleshoot an ostomy.
It is to support a safer transition from hospital to home.
From Assessment to Prescription
Fewer steps between identifying a need and implementing the plan.
Ostoluxe combines specialized ostomy care with advanced-practice provider services.
When an evaluation identifies the need for a different pouching system, appliance, accessory, or other medically necessary ostomy supply, Ostoluxe will manage the corresponding prescription or order directly.
When clinically appropriate, medications related to ostomy function, output, hydration, or other aspects of the patient's treatment plan can also be prescribed and managed; NOT in place of current care team management. This will be closely communicated, provider to provider.
This integrated model can reduce unnecessary delays between clinical assessment, treatment planning, prescribing, and implementation while maintaining communication with the patient's existing healthcare team.

Ostoluxe recognizes that optimal ostomy care is multidisciplinary.
We value collaboration with colorectal surgery, gastroenterology and IBD specialists, WOC nursing, primary care, hospital and discharge teams, dietitians, home health, infusion services, DME suppliers, and other professionals involved in the patient's care.
When appropriate, relevant findings, treatment plans, changes in therapy, and recommendations can be communicated back to the referring or established care team.
Concerns requiring surgical, gastrointestinal, emergency, dermatologic, nutritional, or other specialty management are appropriately escalated.
The goal is coordinated care—not fragmented care.
